Clinical Sciences: Corrección quirúrgica de la ptosis palpebral en el niño: Surgical correction of upper eyerlid ptosis in children |

A study of 114 cases operated due to upper eyerlid ptosis in the months of October 2004 to August 2006 was carried out in the Southern Children Hospital from Santiago de Cuba where the folding of the aponeu-rosis of the elevator muscle of the upper lid (musculus levator palpebrae superioris) was applied as surgical technique. The most frequent age was from 6 to 10 years and the male sex was the most affected. The postoperative clinical course was satisfactory in most of the cases.
Resumen
Se realizó un estudio de 114 casos operados por ptosis palpebral en los meses de octubre del año 2004 hasta agosto del año 2006 en el Hospital Infantil Sur de Santiago de Cuba donde se aplicó como técnica quirúrgica el plegamiento de la aponeurosis del músculo elevador del párpado superior. La edad más frecuente fue de 6 a 10 años y el sexo masculino fue el más afectado. La evolución postoperatoria fue satisfactoria en la mayoría de los casos.

We describe the clinical and histopathologic characteristics of a variant form of the Vernal Keratoconjunctivitis, called "Exuberant pigmented conjunctivitis" and demonstrate the allergic response to environmental particles combined with high altitude, dust, concentrated ultraviolet rays and dryness.

Describimos las características clinicas e histopatológicas de una variante de la Queratoconjuntivitis vernal, llamada “Conjuntivitis Exuberante pigmentada” y demostramos su respuesta alérgica a partículas del medio ambiente combinadas con un clima seco, polvo y gran altitud.

Exenteration is a radical and disfiguring surgical procedure, indicated in cases where malign and benign processes cannot be treated with more conservative treatment techniques. The most common indication for exenteration is malignant neoplasms. The objective of this study is to describe, in detail, the histopathological analysis of the six ex-enteration specimens received between 2000 and 2005 in a reference center for ocular pathology, in Montreal, Canada.
Resumo
A cirurgia de exenteração é um procedimento radical, indicado em casos de pro-cessos malignos e benignos quando técnicas mais conservadoras são insuficientes para o tratamento. A maior parte das indicações de exenteração é atribuída a neoplasias malignas. Este estudo tem como objetivo relatar em detalhe a análise histopatológica dos seis espécimes de exenteração recebidos entre 2000 e 2005 em um serviço de referência de patologia ocular, localizado em Montreal, Canadá.

Clinical Sciences: Trachoma in school children in Botucatu, São Paulo State, Brazil: Prevalence and spatial localization of detected cases |

Purpose: To evaluate the prevalence of trachoma in school children in Botucatu city, state of São Paulo, Brazil and to verify the local spatial distribution of detected cases.
Methods: Cross-sectional studies investigating trachoma cases among school-age children in the city of Botucatu in 2005. A sample size of 2,092 was estimated based on a previous prevalence of 11.2% in the same city, with an estimation error of 10% and a confidence level of 95%. Children were selected by probable cluster sampling. Spatial data were evaluated by CartaLinx (v1.2) and Geoda softwares and data referring to locations of the schools were converted into Thiessen polygons.
Results: The prevalence of trachoma in Botu-catu school children in 2005 was 3.0%. The spacial distribution of detected cases did not show areas with higher concentration.
Conclusion: The prevalence of trachoma among schoolchildren in Botucatu city - São Paulo State, was 3.0% and the spatial distribution analysis did not show areas of higher concentration of cases in this particular city.
Resumo
Objetivo: Avaliar a prevalência de trachoma em escolares da cidade de Botucatu, São Paulo-Brasil, verificando a distribuição espacial dos casos detectados.
Métodos: Estudo transversal, realizado no ano de 2005, com amostra por conglomerado (escolas) de todas as escolas da cidade de Botucatu. A amostra foi constituída de 2.092 crianças, estimada pela prevalência histórica de 11,2% de tracoma em escolares da mesma cidade. Adotou-se erro de estimação de 10% e interval de confiança de 95%. A distribuição espacial foi feita usando a CartaLinx (v1.2) e o software Geoda, segundo a localização das escolas, convertida pelos polígonos de Thiessen.
Conclusão: A prevalência de trachoma em escolares de Botucatu, no ano de 2005, foi de 3,0%. A distribuição espacial dos casos detectados não revelou áreas de maior con-centração da doença.

Diabetic retinopathy (DR) remains the major threat to sight in the working age population. Diabetic macular edema (DME) is a manifestation of DR that produces loss of central vision. Macular edema within 1 disk diameter of the fovea is present in 9% of the diabetic population. Proliferative diabetic retinopathy (PDR) is a major cause of visual loss in diabetic patients. In PDR, the growth of new vessels from the retina or optic nerve, is thought to occur as a result of vascular endothelial growth factor (VEGF) release into the vitreous cavity as a response to ischemia. Furthermore, VEGF increases vessel permeability leading to deposition of proteins in the interstitium that facilitate the process of angiogenesis and macular edema. This review demonstrates multiple benefits of intravitreal bevacizumab on DR including DME and PDR. The results indicate that intravitreal bevacizumab injections may have a beneficial effect on macular thickness and visual acuity (VA), independent of the type of macular edema that is present. Therefore, in the future this new treatment modality could replace or complement focal/grid laser photocoagulation in DME. In addition, in PDR, this new option could be an adjuvant agent to pan-retinal photocoagulation so that more selective therapy may be applied.

Vascular endothelial growth factor (VEGF) plays an important role in many diseases of the posterior segment of the eye that are characterized by macular edema and intraocular neovascularization. Recently, anti-VEGF agents such as pegaptanib sodium and ranibizumab have been shown to be of benefit in the treatment of choroidal neovascularization (CNV) secondary to age-related macular degeneration (AMD). However, in many parts of the world both pegaptanib sodium and ranibizumab are not readily available. Bevacizumab is a humanized recombinant monoclonal antibody against all VEGF isoforms. It has been proposed as an alternate treatment since it is more readily available. Since October 2005, investigators from Pan-American Collaborative Retina Study Group (PACORES) have tretaed patients with different vitreo-retinal pathologies with intravitreal bevacizumab. In our retrospective studies of relatively short follow-up, we have observed both functional and anatomic improvements in an important group of patients with diseases such as CNV secondary to different etiologies such as myopia and AMD, diabetic macular edema, as an adjuvant in proliferative diabetic retinopathy, macular edema secondary to venous vascular occlusions and pseudo-phakic cystoid macular edema. We believe that our results are promising. However, further studies are needed to determine the dose, injection interval, the number of injections and the long term safely, among other issues.
El factor de crecimiento vascular endotelial (VEGF por sus en inglés, Vascular Endothelial Growth Factor) juega un papel importante en muchas enfermedades del segmento posterior que se caracterizan por edema macular y neovascularización intraocular. Recientemente, se ha demostrado la eficacia de fármacos anti-VEGF como el ranibizumab y el pegaptabib sodio en la neovascularización coroidea (NVC) secundaria a la degeneración macular relacionada a la edad (DMRE). Sin embargo en muchas partes del mundo, tanto el pegaptanib sodio como el ranibizumab no se encuentran fácilmente disponibles. El bevacizumab es un anticuerpo monoclonal recombinado humanizado que se adhiere e inhibe todas las isoformas del VEGF. Este se ha propuesto como un tratamiento alternativo ya que es fácil de obtener. Desde octubre del 2005, los investigadores del grupo PACORES (Pan-American Collaborative Retina Study Group) hemos utilizado el bevacizumab intra-vítreo en diferentes patologías vitreoretinianas. En nuestros estudios retrospectivos de seguimiento relativamente corto, hemos observado mejoría anatómica y funcional en un grupo importante de pacientes con patologías como la NVC secundaria a diferentes etiologías incluyendo la DMRE y miopía, edema macular diabético, como adyuvante en la retinopatía diabética proliferativa (RDP), edemas maculares secundarios a oclusiones retinales venosas y edema macular cistoideo pseudofáquico. Creemos que los resultados son prometedores pero se requieren de más estudios para determinar la dosis, el intervalo entre tratamientos, el número de tratamientos y la seguridad a largo plazo entre otras consideraciones.

HSV stromal keratitis is a frequent cause of ocular and visual morbidity. There are three recognized clinical forms: interstitial, disciform (endotheliitis), and necrotizing stromal keratitis, which all differ clinically as well as pathophysiologically. Although treatment for epithelial HSV keratitis has been standardized, it still remains controversial for the stromal form. The goal and challenge in the treatment lies in the balance of the inflammation generated by the host's immune system, and the infection from the virus. Several studies, including the cornerstone HEDS studies, consolidated the benefit of combined corticosteroid and antiviral therapy in the treatment of all forms of HSV stromal keratitis. Most studies used topical trifluridine as antiviral of choice, however several studies have demonstrated that oral acyclovir (or valacyclovir) along with topical corticosteroids is an equivalent and effective therapy, and avoids the often underestimated epithelial toxicity of long-term topical trifluridine. Prophylaxis from recurrences with oral acyclovir (or valacyclovir) is essential in the management of these patients due to the frequent relapses and its consequence on visual prognosis.

Case Report: Suero Autólogo en el Tratamiento del Síndrome de Stevens-Johnson |

We present a case of Stevens-Johnson Syndrome in a 8-year-old boy caused by the intake of barbiturates prescribed for seizures without previous neurological history. This muco-cutaneous syndrome produces severe lesions in the conjunctival mucosa causing blindness in some cases. The boy was treated with autologous serum eye drops soon after hospital discharge for toxic epidermal necrolysis. He demonstrated improving corneo-conjunctival epithelization, diminishing neovascularization and avoidance of the formation of new conjunctival synechiae. Here we demonstrate one of the indications of autologous serum eye drops in the alteration of the ocular surface.
RESUMEN
Presentamos un caso de Síndrome de Stevens-Johnson en un niño de 8 años de edad, causado por la ingesta de barbitúricos prescriptos por crisis convulsivas sin antecedente neurológico previo.
Este síndrome muco-cutáneo causa severas lesiones en la mucosa conjuntival, llegando incluso a la ceguera en algunos casos.
El niño fue tratado con gotas de suero autólogo en ambos ojos tan pronto fue dado de alta del hospital donde estuvo internado por la epidermolisis necrótica, mejorando la epitelización córneo conjuntival, disminuyendo la neovascularización corneal y evitando la formación de nuevas sinequias conjuntivales. El niño pudo terminar el año escolar.
En esta comunicación enfatizamos las indicaciones del suero autólogo en las alteraciones de la superficie ocular.

Article: Cirugía de Catarata y su Anestesia: Tópica vs. Tópica e Intracameral |

Objective: To evaluate the efficacy of topical anesthesia and topical + intracameral anesthesia in phacoemulsification of different opacity grade cataracts. Material and Methods: 128 patients underwent with phacoemulsification and foldable intraocular lens implantation. The patients were divided into two groups: topical anesthesia only (n=64) and topical + intracameral anesthesia (n=64) with preservative free 1% lidocaine added to the infusion solution. Cataract opacity grade, intra-operative and immediate post-operative pain as well as vital signs were recorded.
Results: Mean age of 68.4 years ± 2.7 D (49-90), 67 females (52.34%) and 61 males (47.66%). Cataract opacity grades were as follows: II (18%), III (61.7%), IV and greater (20.3%). Thirteen patients experienced mild pain: 9 (7%) in the topical anesthesia group during nuclear rotation and 4 (3%) in the topical + intracameral anesthesia group during insertion of the phaco needle into the anterior chamber. (p= 0.14) Vital signs were stable.
Conclusion: Topical anesthesia is both safe and effective in phacoemulsification. Topical + intracameral anesthesia is more appropriate for cases with cataracts of greater density.
RESUMEN
Objetivo: Evaluar la efectividad de la anestesia tópica sola y anestesia tópica e intracameral en diferentes grados de cataratas tratadas con facoemulsificación.
Material y Método: Se revisaron 128 pacientes con catarata y sometidos a facoemulsificación e implante de lente intraocular plegable. Se dividieron en dos grupos: grupo de anestesia tópica (n=64) y grupo de anestesia tópica e intracameral (n=64) con lidocaína al 1% libre de conservadores administrada a la infusión de irrigación. Se registraron tipo de catarata, dolor trans y post operatorio inmediato y signos vitales.
Resultados: La edad media fue de 68.4 ± 2.7 D (49-90), 67 mujeres (52.34%) y 61 hombres (47.66%). Las cataratas se distribuyeron en los siguientes grados: II (18%), III (61.7%), IV y mayor (20.3%). Trece pacientes presentaron leve dolor, 9 (7%) del grupo con anestesia tópica al rotar el núcleo en cataratas grado IV y 4 (3%) en el grupo de tópica e intracameral, durante la introducción de la pieza de mano a la cámara anterior. (p=0.14) Signos vitales eran estables.
Conclusión: La anestesia tópica es segura y efectiva en facoemulsificación, la anestesia tópica e intracameral es mejor en casos de catarata de mayor densidad

Empleando la experiencia del Grupo de Micromecánica y Mecatrónica, GMM, del Centro de Ciencias Aplicadas y Desarrollo Tecnológico, CCADET, de la Universidad Nacional Autónoma de México, UNAM, que desde 1999 trabaja con el desarrollo de sistemas micromecánicos y sus aplicaciones, se presenta en este trabajo el diseño y construcción de un prototipo de prótesis de substitución sensorial visual para pacientes ciegos a partir de un sistema de barras efectoras, activadas por electromagnetismo, que se desplazan de manera lineal ejerciendo presión sobre la epidermis del paciente, generando así una matriz de sensaciones puntuales (tres por tres), que serán la base para la reconstrucción de imágenes.
Actualmente se trabaja con el desarrollo y la caracterización de las sensaciones generadas por diversas geometrías de las barras efectoras y el impacto de la separación de las mismas para que el sujeto alcance a percibir estímulos independientes; la importancia de lo anterior radica en establecer la mayor relación de número de puntos por unidad de área, dado que al aumentar el grado de resolución de la matriz es posible reconstruir escenas visuales de mayor complejidad. Con estos resultados se espera determinar las geometrías y separación de barras óptimas del sistema.

Case Report: Spontaneous globe luxation in a 4-year old |
|

Purpose: To report a case of spontaneous globe luxation in a 4-year old with unknown cause. Methods: Case Report. Results: The patient underwent globe restitution using topical anesthesia, and no visual complications were associated with the procedure. Post procedure CT scan revealed normal orbital structures and contents. Conclusion: Spontaneous globe luxation is a rare entity, generally associated to thyroid orbitopathy or floppy eyelid syndrome and shallow orbits. Techniques to manage this dramatic event range from bare manual restitution or the same technique aided by paper clip to lateral tarsorrhaphy.

Resumen
En este trabajo se evalúo la eficacia de la utilización de la AGUJA EN PATÍN DE NIEVE que es utilizada de forma tradicional en la cirugía general y ortopédica y fue usada por primera vez en la cirugía oftalmológica en la corrección quirúrgica de ptosis palpebral congénita severa. Fueron intervenidos quirúrgicamente 112 ojos de 89 niños entre 2 y 14 años de edad. Durante la cirugía y en el postoperatorio se evalúo el tiempo quirúrgico empleado, el sangrado, el edema y el riesgo de infección. Durante nuestro trabajo no se identificaron complicaciones importantes que comprometieran el resultado final estético deseado.

OBJECTIVES: Analyze the characteristics and results in the Retinopathies of the Prematurity (ROP) Blindness Prevention Program in the given period. METHOD: Retrospective study of Series screening babies to present ROP risk factors, among 1998-2005 included. RESULTS: In total was screening 4396 children, the incidence annual average was of 16/100 cases included in the screening protocol and the calculated prevalence was 0.3/10,000 infants The age average and weight in the series were of 30.1 weeks and 1702 grams of weight. The 11.2% of screening babies (493) develops ROP and 2% (79) received treatment with cryotherapy (69%) while the remaining 31% with photocoagulation with laser diode and green-blue by means of indirect binocular ophthalmoscope, 15% of the treaties by means of cryotherapy and 4% of the photocoagulates developed derived complications of proceeding. CONCLUSIONS: The positive results as for the identification and treatment of the ROP by means of our program suggests their continuation and extension or application in environments with socioeconomic similarity to ours.
RESUMEN
OBJETIVOS: Analizar las características y los resultado del programa Prevención de la Ceguera por Retinopatía de la Prematuridad (ROP) en el periodo dado. METODO: Estudio Retrospectivo de Serie de Casos pesquisados por presentar factores de riesgo para la ROP, en el periodo comprendido entre los años 1998-2005 incluidos. RESULTADOS: En total se pesquisaron 4396 niños, La incidencia anual promedio fue de 16/100 pesquisados y la prevalencia calculada del 0.3/10,000 infantes. Las medias de edad y peso en la serie fueron de 30.1 semanas y 1702 gramos de peso. El 11.2% de los pesquisados (493) desarrollo ROP y el 2% (79) recibió tratamiento. De estos, el 69% se trató con crioterapia, mientras el restante 31% con fotocoagulación con láser diodo y verde-azul mediante oftalmoscopio binocular indirecto. El 15% de los tratados mediante crioterapia y el 4% de los fotocoagulados desarrollaron complicaciones derivadas del proceder. CONCLUSIONES: Los resultados positivos en cuanto a la identificación y tratamiento precoces de la ROP mediante nuestro programa sugiere su continuación y extensión de su aplicación en entornos con similitud socioeconómica al nuestro.

Article: Manifestaciones Oculares en Algunas Enfermedades Reumáticas en el Niño |

Se realiza un estudio descriptivo prospectivo con un grupo de 45 pacientes remitidos por primera vez a nuestro servicio, portadores de algunas de las enfermedades reumáticas. De ellos 37 (82%), presentaban o habían presentado en algún momento uveítis anterior u otros tipos de alteraciones del segmento anterior. Se halla igual número de casos para ambos sexos y el 20% era menor de 5 años. Estos pacientes en su gran mayoría no presentaron, ni síntomas ni signos que hicieran sospechar que se estaba desarrollando una enfermedad ocular. La mayoría tenía diagnóstico de Artritis Idiopática Juvenil. Los hallazgos más relevantes en la lámpara de hendidura fueron la presencia de pigmentos y sinequias. Todos los casos recibieron tratamiento local, asociado o no a tratamiento sistémico (esteroide y/o inmunosupresor). La evolución fue satisfactoria en la mayoría de los pacientes.
